The Center is conveniently located, just two turns from the interstate.

From Interstate 5 (I-5) take exit 27. At the end of this exit is a light. Make a right turn, East bound and travel for ~2 miles.

The office is the third building, past the intersection of Black Oak and Barnett Road, one block before the Rogue Valley Medical Center and across the street from the Eye Center. There are two banks on the Black Oak corner, at the light. We are located on the right side of Barnett Road when traveling East bound.

Our driveway is just past the Minute Market entrance. The Center of Health sign is directly in front of the Center.
